What to bring to your first appointment
We recommend that you try to arrive a bit early for your first appointment to make sure you have time to get settled, meet our team, and complete some forms. It will speed up your new patient intake if you could bring the following information with you:
- Your current doctor’s name and contact info
- A list of your current prescriptions
- Any information on allergies or existing medical conditions
- Your dental health insurance information

Complete forms in advance
To save time during your first visit, you can download and complete the below forms before your appointment. Otherwise you’ll likely have to complete these in-person when you arrive.

Insurance
We provide direct billing with most insurance plans. Please note that you are responsible for any uninsured costs. Our front desk team can help you understand your coverage and treatment plan.
Canadian Dental Care Plan (CDCP)
The CDCP is a federal government program designed to cover most dental care treatment costs for Canadians who qualify. If you’ve applied and been approved for coverage you can use this plan at our clinic. Learn more about the CDCP.
Financing
We offer a third party financing option. If you are interested in this option or have questions about it, we can give you more information about it at your appointment.
Balance owing For any portion of your treatment costs not covered by your insurance, our payment options include: Cash, Debit, Cheque, Visa, MasterCard, American Express and payment plans.
